This application is an internal proof of concept operated by Unite. It is made available only to authorized Unite personnel who are retrieving their own health records. It is not a general-availability product and carries no service commitment.
You may use this application only to request records about yourself, and only with your own explicit authorization. Requesting records about another person through this application is prohibited.
All queries are submitted under the Individual Access Services (T-IAS) purpose of use.
This application does not submit Treatment-purpose queries and must not be used as a substitute for
one.
Retrieved records are handled as described in the POC Privacy Notice, including encryption at rest, restricted access, a 30-day maximum retention period, and on-demand deletion.
Records are supplied by third-party healthcare organizations and networks. Unite does not warrant that retrieved records are complete, current, or accurate, and they must not be relied on for clinical decisions.
Unite may disable access at any time. On conclusion of the evaluation, all retrieved records are deleted.
